Springfield Local Schools, in partnership with ProMedica, will hold a Back to School COVID-19 vaccination clinic on Wednesday, August 17, from 4 to 6 p.m., at the district administration building, 6900 Hall Street, Holland.
COVID-19 vaccination clinic participants (age 5 or older) may:
•Schedule an appointment at gettheshot.coronavirus. ohio.gov/ to receive a first dose of either the Pfizer or Moderna vaccines. This clinic will offer first, second and booster dose appointments (first dose recipients will need to schedule their own second dose appointment through an area vaccine provider).
•Those requesting a booster vaccine are reminded to bring the vaccination record card provided at the previous administrations of the vaccine (required– if unable to locate, contact amandsaucedo@springfield-schools.org).
•Individuals wishing to arrive without an appointment may do so after 5 p.m.
•A legal guardian must complete a consent form. All minors must be accompanied by an adult. Anyone under the age of 18 or arriving without a guardian to complete and sign a consent form will not be able to receive the vaccine.
