Springfield Local Schools will hold a back to school vaccination clinic on Wednesday, August 16, from 9 a.m. to 3 p.m., at 6900 Hall Street.
Participants can receive prizes and register to win a $50 gift card.
Following is a list of immunizations required for school children by the State of Ohio.
To provide convenience for families, Springfield Local Schools has partnered with Shots 4 Tots-n-Teens to provide this in-district clinic.
Families are asked to bring their insurance card (if applicable) and a paper shot record.
There is a $21.25 administration fee if a child has no insurance, although no child will be turned away for inability to pay.
Kindergarten (prior to entry into kindergarten) include: Hepatitis B– three doses. The second dose must be at least 28 days after the first.
The third dose must be given at least 16 weeks after the first dose and at least eight weeks after the second dose.
The last dose in the series (third or fourth dose) must not be administered before age 24 weeks. DtaP– completed series of four doses with the last dose on or after the fourth birthday.
Polio– completed series of at lest three doses with the final dose on or after the fourth birthday. MMR– two doses with the first dose administerd on or after the first birthday. Varicella (chicken pox)–two doses with the first dose given on or after the first birthday.
Springfield Middle School (prior to entry into the seventh grade) include: TdaP– one dose must be administered on or after the
10th birthday. Meningococcal (serogroup A, C, W and Y)–first dose prior to entry.
Springfield High School (prior to entry of the 12th grade) include: Meningococcal (serogroup A, C, W and Y)–completed series of two doses prior to entry. If the first dose was administered on or after the 16th birthday, a second dose is not required.
