Important Reminders from Springfield Middle School:

Attention ALL

7th Grade Parents

In case families missed or overlooked the important reminders that we shared, there are two vaccines that are required for seventh grade enrollment: the Tdap and meningococcal. Thus far, Springfield Middle School nurse Maureen has already received documentation from half of the current seventh grade, which means that we need assistance from many, many more families to ensure that students’ medical records are up-to-date.

Nurse Maureen does have access to the Ohio database that physicians/clinics use, and is able to track down secure records using this system. She also accepts paper records for those who may have received vaccinations from out-of-state locations. If you have a student in grade seven at SMS, who is yet to receive these required vaccinations, please contact their pediatrician to schedule an appointment or consider using Shots4Tots/Teens through the Toledo Lucas County Health Department (TLCHD) (lucas countyhealth.com/child-immu nizations/).

Those seeking service through the Shots4Tots/Teens program can reach the TLCHD by calling 419-213-4100. From their menu, select option one. Student’s shots/records need to be up-to-date with SMS by September 15. Anyone needing a paper copy of their child’s immunization record (required by the health department at time of appointment) may call our SMS nurse at 419-867-5767 or direct their student to stop in the SMS main office.
